Insurance

Florida Digestive Health Specialists participates with most insurance plans. Prior to any visit, you should contact your insurance provider to verify accepted plans. We ask that you please bring your insurance card(s) with you to every visit.

Medicare Patients: 

As participating providers of Medicare, we will file claims on your behalf. Under the Medicare program, the insured is responsible for a 20% co-payment after satisfying their Medicare deductible. As a courtesy, we will also file a secondary insurance plan unless you request otherwise. 

Managed Care/Private Insurance 

We participate in most major managed care (HMO, PPO) plans for patient convenience. If a referral or pre-authorization is required before your visit or treatment, please ensure this is completed with the insurance plan provider or primary care physician before your appointment. Otherwise, you will be responsible for the entire cost of the visit or procedure. 

Patient Financial Responsibility  

Co-pays, co-insurance, deductible payments, and self-pay fees are the responsibility of the patient and are collected at the time of service. We accept most major credit cards, cash, and personal checks for your convenience.
